Purchase Orders

We sell directly to schools, hospitals, and government institutions and gladly accept purchase orders. To place an order with a purchase order, please send them via email or fax to:

1. Email scanned POs to: info@mosaicweightedblankets.com
2. Fax to: (512) 597-0774 

PURCHASE ORDERS MUST INCLUDE:

1. An email and phone number
2. Company or school name/number
3. Bill to info - address & phone number.
4. Ship to address and phone number (with attention to whom is should be addressed to).
5. Email address of person responsible for processing & paying invoices.
6. A purchase order number.
7. The quantity, product number, and a brief item description of the weighted products you are purchasing.
8. A purchase order total amount
9. An authorized signature.
